Professional referrals
For social workers, discharge coordinators, GPs, district nurses and case managers. Send it over and a named coordinator will come back to you with a clear answer on capacity.
2-hour acknowledgement
Every referral is acknowledged within two hours during working time, with a named coordinator attached to it.
A straight yes or no
We tell you honestly whether we have capacity. If we cannot take it on, you will know quickly enough to place it elsewhere.
Same-day mobilisation
Where it is urgent, rapid response puts a carer in place within two to four hours, including out of hours.
Documentation ready
Insurance, policies, CQC status and KPI data available the same working day. See the pack →
From referral to care in place
-
1. You send the referral
Using the form here, by email, or by phone if it is urgent. Include the discharge summary and medication list where you have them.
-
2. We acknowledge within 2 hours
In working time. You get a named coordinator and a reference, not an automated reply.
-
3. Capacity confirmed
A clear answer on whether we can take it, when we could start, and what we would need first.
-
4. Assessment and start
Assessment within 48 hours as standard, or same-day where rapid response applies. Care plan shared with you and the GP.
-
5. Feedback loop
You receive an update after the first week and are copied into the six-week review outcome where appropriate.
Urgent referral?
Do not use the form. In office hours call 020 3640 7722. Outside them, our 24/7 lines are 07983 163392 or 07399 472345 — answered by a real person who can reach the on-call team.
